Dear Mr. Bisnett,
I want to thank you for all the wonderful things you and your staff have done for Aaron during the time that he was at Pride, but most of all for the care and concern that Staff had for him during the past few months; and allowing him to return home to be near his friends - his family, the last few days. I know it meant a great deal to Aaron as it did to me. These are the memories I will cherish as I remember what you have done for him.
Aaron had such a spirit about him. He enjoyed life and was a survivor. The good memories will continue for a long time to come. What a celebration we had with his Pride family attending! Pride did so much for Aaron I can't even imagine what life for him or me would have been like without them.
Many of Aaron's relatives, friends and the staff from Pride donated money in memory of Aaron. It would be my wish that you accept this and use it for other residents within Pride that don' t have the resources for some basic need or for something a little extra. Most of Aaron's other possessions have been given to Pride so that they too can be used and Aaron remembered.
Again, I want to say Thanks for everything. Please let everyone know how I feel about them.

Pride, Inc. 401(k) Profit Sharing Plan Highlights

Eligibility
All Contributions: You must work 1,000 hours within 12 months and be at least age 21 to contribute to the Plan.
|
|
Initial Entry Dates
All Contributions After completing the eligibility requirements, you will enter the Plan on the following January 1 or July 1, whichever comes first.
|

Contributions
Employee Pre-Tax Contribution Limits: Up to 100% of your compensation, not to exceed $15,500. If you are at least age 50 by December 31, you may contribute an additional $5,000 in catch-up contributions (2008 limits).
|

Changes to Contribution Amounts: Changes are permitted quarterly upon written notification to the Employer.
|
|
Employer Matching Contributions: Your Employer will match 100% on the first 4% of compensation contributed.
|
|
Employer Discretionary Contributions: You must work 1,000 hours during the year and be employed on June 30 to receive the discretionary contribution, if one is made.
|
|
Rollover Contributions: A tax-free rollover from a former Employer's qualified retirement plan or a traditional IRA is permitted prior to satisfying the Plan's eligibility requirements.
|
|
Vesting: You are always 100% vested in your contributions and roll-over balances. Employer matching and discretionary contributions vest according to your "years of service." A Year of Service is a year that you work at least 1,000 hours.

Distributions

BOX TOPS FOR EDUCATION
The school at Pride Manchester is asking that everybody save their box tops from General Mills products. The box tops are then turned in and vouchers are received to be used to buy school supplies, go on field trips/activities, or whatever the children desire. There is a drop off spot at the Missouri office and at Pride Manchester.

Other Info/Updates
The Business Office has adjusted all mileage back to August and provided a retro payment where budget allows. The current mileage reimbursement has been changed to $.50/mile, where budget allows.
Please verify that the address printed on your earnings statement is current. This is the address where your W2 will be sent in January 2010. If your W2 needs to be reprinted for any reason, there is a $15 charge. Contact Human Resources to update an incorrect address.
You are required to complete a new W-4 for 2010 if:
-You claimed “Exempt” status on your 2009 W-4*
-You claimed 10 or more exemptions on your 2009 W-4
-Your exemptions have changed since you filed your last W-4
*Note: If a 2010 W-4 claiming “Exempt” status is not submitted by February 16, 2010, the Federal filing status will default to “Single, with zero withholding allowances”.
